Patient History Form
Please complete either version of our complete history form before coming to our offices. This can be done by clicking either link below, saving the document to your desktop, printing out the form, completing it in ink, then either mailing it to us (2905 San Gabriel # 306, Austin, TX 78705) or fax it to 512-479-6002, prior to your appointment to allow more time for us to spend with you. If that is not possible, simply bring the completed form with you to your scheduled appointment.

Alternately, the MS Word Version may be completed electronically. To do so save the document to your Desktop or another location of your choice. With the document open, complete the form by typing in the spaces provided, saving your changes, then attach the completed form and email it to us before your scheduled appointment date. 

This not only allows you to keep a copy of your history and makes changes as necessary, but also will allow us time to spend more quality time with you.

  TIPS ON COMPLETING YOUR FORM: The lines have been created using the underline function and the tab key. To maintain the size of the document please use the delete key to remove blank lines if you have entered a great deal of information.
