Medical Services and Rates (NEW
RATES as of 9/22/06)
The fees for service listed below have been established without regard to your
medical insurance status. While annual exams include some laboratory fees*,
office visits do not. Laboratory and diagnostic test fees for office visits or
those not included in the annual exam will be billed directly to your insurance
company for your convenience if you have a medical insurance policy. If you do
not have medical insurance and laboratory or diagnostic studies are needed, we
can offer some tests to you at the discounted rates you see here.
Fees for service are due at the time of the visit and exceptions for
payment in full cannot be made. If you do not have medical insurance
laboratory/diagnostic fees will also be due at the time of service.

Adult Exam
(25-65 yrs) |
Complete medical history and physical examination as well as
body fat composition assessment, complete blood panel, oxygen saturation, PAP or PSA (men), pulmonary
function and urinalysis. Patients over 45 years will also have an EKG and
occult blood in stool evaluated annually.
Additional laboratory/diagnostic tests may be ordered if
needed or desired.*
All new patients and patients
who have not been seen in over 2 years need to complete a history form.

May include, but is not limited to: Bio-identical hormones, cholesterol (lipid) management, depression/anxiety, dietary/nutritional issues (IBS, gluten sensitivity), herbal/dietary supplement use, meditation/Mind-Body-Spirit techniques, Menopause/Perimenopause, metabolic conditions (chronic fatigue, fibromyalgia, hypothyroidism), panic attacks and weight loss/management counseling.
Laboratory tests not included.
